Everyone. I understand that all medicine is risk management, and if there is a potential benefit for the patient then there might be an acceptable risk in giving medication, particularly as this medication has never been licensed for this disease, and when the medication was being trialled it is a known side effect. Had it improved his quality of life it might have been a risk worth taking. There was no chance this would ever improve his quality of life. It had a massive negative impact.

Secretagogues Sulfonylureas have been the mainstays of oral hypoglycemic therapy for the past 40 years. Sulfonylureas help to increase the secretion of insulin but have no effect on insulin sensitivity. The main side effects of this class of agents is hypoglycemia and weight gain. Primary failure of this drug is due to insulin insufficiency. Secondary failure may include 19, 25, 28, ; poor dosing, lack of physical activity and obesity. The newer class of secretagogues includes Prandin and Starlix. These medications also stimulate pancreatic insulin release, but in contrast to the sulfonylureas, they start acting within 30 minutes and are cleared by the body within 3 to 4 hours. These drugs should be taken immediately prior to each meal. The main advantage of these medications is that 48 ; there is more flexibility in timing of meals, less weight gain and hypoglycemia. Biguanides Metformin ; Biguanides were banned in the United States during the 1970's because they were linked to lactic acidosis. A safer biguanide, metformin, has been released brand name Glucophage ; and the risk of lactic acidosis appears to be minimal. This drug works through decreasing the amount of glucose released from the liver. This drug may also aid 25, 28-30 ; in weight reduction and improvement of lipid profiles. Thiazolidinediones Actos, Avandia ; Actos and Avandia work by increasing insulin sensitivity. They lower blood sugar through improvement of target cell response to insulin. Persons on these drugs need baseline liver function testing and should be monitored for any signs of hepatic problems. Patients should also be evaluated for any signs of edema or weight gain since they medications can increase fluid retention. These drugs should be used with caution in people with 25, 28, 30, ; congestive heart failure or on insulin therapy. Glucosidase Inhibitors Precose, Glyset ; Precose and Glyset are used to control digestive enzymes, delaying the digestion of complex carbohydrates. This interference can prevent a dramatic rise in blood sugar after 28-30, 47, 48 ; eating a meal. Combination Medications To improve efficacy and simplify medications regimens, three of these diabetes drugs are now combined. These medications combine Metformin plus Glyburide or Glucotrol and 48 ; met-formin plus Avandia and glucotrol.
Calorie restriction, exercise and weight reduction. Although type 2 diabetes mellitus can be improved with these lifestyle measures, long-term compliance is difficult, so most patients will eventually require oral medications. Some medications for type 2 diabetes mellitus work by increasing production of insulin in the pancreas. Examples of these include Micronase, Glucotrol, and Prandin. Other medications, such as Avandia, Actos and metformin Glucophage ; work by decreasing insulin resistance, which improves the ability of muscle and other organs to take up glucose from the bloodstream. Another oral agent, acarbose Precose ; lowers blood sugar by slowing down digestion of complex carbohydrates in the small intestine. Insulin is prescribed for individuals with type 2 diabetes mellitus only when they are not adequately controlled on oral medication. Risk factors for developing type 2 diabetes mellitus include family history, obesity, aging, prior history of gestational diabetes a transient form of diabetes induced by pregnancy ; , and impaired glucose tolerance, which is present when blood sugars are elevated but fall below the cutoff for.

Since highly active antiretroviral therapy HAART ; became widespread, age at seroconversion and sexual risk factors no longer strongly affect disease progression in patients with HIV infection. However, HAART has had less effect on the prognosis of infected injecting drug users, whose risk of progression to AIDS and death is significantly higher compared with men who have sex with men. Dr. Kholoud Porter and other members of the CASCADE Concerted Action on SeroConversion to AIDS and Death in Europe ; collaboration analyzed data for 22 cohorts of people with HIV-1 in Europe, Australia and Canada [and published these findings in the October 18 issue of The Lancet 362, p. 1267, 2003 ; . Included were 7740 individuals with a known date of seroconversion. Compared with the pre-HAART era, the overall hazard ratio HR ; for progression to AIDS was 0.46 in 1997 and 0.13 in 2001. Similarly, the HR for mortality had dropped to 0.47 in 1997 and to 0.16 in 2001.The effect of age on progression to AIDS was attenuated over time. Before 1997, the HR was 1.27 for each decade increase in age at seroconversion. Between 1999 and 2001, HR declined to 1.03 for each 10-year increase in age. However, mortality risk remained higher in those over 45. Over time, injection drug use replaced homosexual male sex as the most important prognostic factor related to exposure. Compared with homosexual men, the HR for progression to AIDS was 0.74 among injecting drug users before 1997. Between 1999 and 201, the HR was 2.76. Mortality risk followed a similar pattern. In 1997, risk of death was similar in homosexual men and injecting drug users. Between 1999 and 2001, HR was 4.59 for male drug users and 2.99 for female drug users. Multiple factors may be responsible for these shifts, Dr. Porter's group notes. Increased mortality in drug users could be due to competing non-HIV-related mortality, less access to HAART, and worse adherence. Meanwhile, the reduction in incidence of Kaposi's sarcoma likely reduced rates of disease progression and death among men who have sex with men.

| POSTPARTUM M A T EWES from an attempt to breed Precose ewes twiceyearly and indicated difficulty in getting spring-lambing ewes to mate and conceive following lambing. There have been several attempts to speed the reproductive rate by breeding ewes as soon after lambing as possible Copenhaver and Carter, 1964; Shelton and Huston, 1968b; Usakava and Fudelj, 1941 ; . These studies have involved mating ewes at all times during the year and results have indicated lambing intervals of over 200 days with seasonal differences in the postpartum intervals. It has generally been found that ewes of fine wool and Dorset breeding are more adaptable to yearlong breeding, but even they are more fertile if mated during the fall as reported by Shelton and Morrow 1965 ; with Rambouillet ewes. This study was conducted with ewes of Dorset, Rambouillet and Dorset x Rambouillet crossbred breeding to characterize the mating and lambing behavior o.f such ewes under a fixed twice-yearly lambing schedule. Materials and Melhods The experimental flock consisted of 60 Dorset, 60 Rambouillet and 62 Dorset x Rambouillet ewes assembled in three approximately equal replicates at 6-month intervals. Sixty-two 20 Dorset, 20 Rambouillet and 22 Dorset x Rambouillet ; ewe lambs born during the spring of 1963 were purchased at 4 to months of age and grown until the spring of 1964 at which time they were mated to lamb first during the fall of 1964 at approximately 18 months of age ; . The second replicate of 60 ewes representing 20 ewe lambs of each breed group were born during the fall of 1963, purchased at 4 to months of age and bred during the fall of 1964 to lamb at about 18 months o.f age also. The third replicate represented 20 ewe lambs of each breed group also and followed the same schedule as the second replicate but 6 months later. The ewe lambs were purchased purposely so that no more than 10 ewes came from any one flock thus some reasonable sampling was done. Throughout the experiment the breed groups were in.

PRECOSE is available as 25 mg, 50 mg and 100 mg tablets for oral use. The inactive ingredients are starch, microcrystalline cellulose, magnesium stearate, and colloidal silicon dioxide. CLINICAL PHARMACOLOGY Acarbose is a complex oligosaccharide that delays the digestion of ingested carbohydrates, thereby resulting in a smaller rise in blood glucose concentration following meals. As a consequence of plasma glucose reduction, PRECOSE reduces levels of glycosylated hemoglobin in patients with type 2 diabetes mellitus. Systemic non-enzymatic protein glycosylation, as reflected by levels of glycosylated hemoglobin, is a function of average blood glucose concentration over time. Mechanism of Action: In contrast to sulfonylureas, PRECOSE does not enhance insulin secretion. The antihyperglycemic action of acarbose results from a competitive, reversible inhibition of pancreatic alpha-amylase and membrane-bound intestinal alpha-glucoside hydrolase enzymes. Pancreatic alphaamylase hydrolyzes complex starches to oligosaccharides in the lumen of the small intestine, while the membrane-bound intestinal alpha-glucosidases hydrolyze oligosaccharides, trisaccharides, and disaccharides to glucose and other monosaccharides in the brush border of the small intestine. In diabetic patients, this enzyme inhibition results in a delayed glucose absorption and a lowering of postprandial hyperglycemia. Because its mechanism of action is different, the effect of PRECOSE to enhance glycemic control is additive to that of sulfonylureas, insulin or metformin when used in combination. In addition, PRECOSE diminishes the insulinotropic and weight-increasing effects of sulfonylureas.
