In adults, there are promising reports about its use in a nonneurogenic overactive bladder OAB ; . We evaluated the safety and results of intradetrusor injection in children with a non-neurogenic OAB. Material and methods Five girls and six boys with a mean age of 10 years, were included. All had therapy resistant daytime incontinence, detrusor overactivity and normal emptying. They were treated for more than 3 years and all available treatments had failed urotherapy, anticholinergics, neurostimulation, voiding school ; . Intradetrusor injection was performed cystoscopically under general anesthesia. Two patients received 125 Units Dysport, nine patients 250 Units. Evaluation was done using frequency-volume charts and urodynamics. Results No patient had general side effects. There was a mean increase in functional bladder capacity of 83% and a decrease in amplitudo of overactive bladder contractions of 74%. Patients had a marked decrease in urgency symptoms. Residual urine was less than 25 in eight and less than 50 ml in two more patients. One girl needed to perform intermittent catheterisation during two weeks. Two patients had two injections because of a partial response. 82% of patients were very satisfied without having to use anticholinergics. The effect lasted eight months in one girl. Conclusion We conclude that this treatment modality can be used safely in pediatric patients with a therapy resistant OAB, with good short-term results. However, more data are needed to study the optimum dosage in these patients and the long term effects. Post-void residual urine is a frequent finding postoperatively which can make intermittent catheterisation necessary. Parents and children should be aware of this risk before treatment.
